Consent to Use of Automated Clinical Note Taking

I consent to Midi Health’s use of a third-party artificial intelligence clinical note taking assistance tool (“AI tool”).  This tool aids in creating accurate clinical notes from your conversations with your clinician and allows us to focus more on you, the patient. 

What is the tool?

• Midi utilizes an AI tool that listens to the conversation during a medical consultation and generates a written summary or “note” based on that conversation.  The note is then reviewed, updated as necessary and approved by your clinician.    

How will this affect you?

• The AI tool does not interact with you directly.  It merely listens to the conversation and creates a summary. This allows your clinician to focus more on you during the visit and less on taking notes.

Data Privacy and Confidentiality: 

• While the AI tool assists in the process of note-taking and chart generation, it's important to note that the system may process sensitive health information provided during your consultation. 
• The information processed by the AI tool is strictly used for the purpose of your ongoing medical care and is subject to strict data privacy regulations to ensure your information remains confidential.

Potential Risks: 

• While the AI tool is designed to improve the efficiency of your medical care, there are potential risks associated with its use. These include, but are not limited to, inadvertent errors in transcription or interpretation of your health information, and potential, though highly unlikely, breaches of data security. Your participation in the use of the AI tool is completely voluntary.  By consenting to this form, you understand the above explanation regarding the AI tool, its uses, potential benefits, and risks and consent to the use of the AI tool in the process and documentation of your health information.  

If you have any questions or concerns regarding the use of the AI tool or would like to opt out of use of the AI tool during your visit, please inform your clinician before the start of your visit.
