Consent to Audio Recording and AI-Assisted Clinical Note Taking

I consent to Midi Health’s use of a third-party artificial intelligence clinical note taking assistance tool (“AI tool”), including its creation of an audio recording of my visit to generate a transcript and draft clinical notes that are subsequently reviewed and approved by my clinician. 

What is the tool?

  • Midi uses an AI tool that creates an audio recording of the conversation during a medical consultation and uses it to generate a transcript and draft clinical note.  The note is then reviewed, updated for accuracy and completeness as necessary, and approved by your clinician. This tool aids in creating accurate clinical notes from your conversations with your clinician and allows us to focus more on you during the patient visit.   

How will this affect you?

  • The AI tool does not interact with you directly.  It documents the transcript and draft clinical note. This allows your clinician to focus more on you during the visit and less on taking notes.

Data Privacy and Confidentiality: 

  • While the AI tool assists in the process of note-taking and chart generation, it's important to note that the system may process sensitive health information provided during your consultation. 
  •  Midi uses the transcript for charting and treatment and may also use it for other purposes permitted by the HIPAA Privacy Rule, such as quality assessment and improvement, training, auditing and compliance, and other health care operations. The audio recording, transcript and clinical note are protected as described in Midi's Notice of Privacy Practices and as required by applicable law.

Verification: 

  • AI-generated transcripts and draft notes may contain errors. Your clinician reviews and approves the final clinical notes before they are finalized and incorporated into the medical record.

By acknowledging this consent when scheduling, you understand the above explanation regarding the AI tool, its uses, potential benefits, and risks and consent to its use.
