Credit Card on File Policy

Midi Health has adopted this Credit Card on File Policy.  At the time of registration, we will request your credit card information. Your credit card numbers will be encrypted and stored securely off-site.  For your protection, no credit card numbers will be stored at our practice.   Midi Health personnel will not have access to your card; only the last 4-digits of your card number will be shown in our system.

You may be asked to use your credit card on file to pay co-pays at the time of each telehealth visit, and Midi Health may use your credit card on file to pay any account balances after claims adjudication by your insurance company.  Once your insurance company has processed your claim, it will send an Explanation of Benefits (EOB) both to you and to Midi Health as your care provider.  The EOB shows the amount of your total payment responsibility.  You typically receive the EOB before we do, so if you disagree with the patient responsibility amount owed, it is your responsibility to contact your insurance company immediately.  Once we receive the EOB, we will charge your credit card for any outstanding balance that is your responsibility.

FREQUENTLY ASKED QUESTIONS

Why does Midi Health require me to keep a credit card on file? To assure payment.  Midi Health requires you to maintain a card on file.  By keeping a credit card on file, you will be able to:

  • Pay co-pay and deductible amounts, and any balances for non-covered items and services conveniently when due
  • Make payments automatically using your credit card of choice
  • Receive notifications and receipts sent via email

Please note that all of your rights with respect to the use of your credit card remain in effect. This policy in no way prevents you from being able to dispute a charge or question your insurance company’s determination of payment. 

How will Midi Health use my credit card on file?  Your credit card on file will be used by Midi Health only for the following reasons:

  • Visit payments not collected from you at the beginning of the visit
  • Insurance discrepancies
  • Outstanding balances after insurance adjudication
  • Cancellation fees     
  • Payment for products purchased through Midi Health

What is a deductible and how does it affect me? An annual deductible is the dollar amount you must pay out of pocket during the year for medical expenses before your insurance coverage begins to pay. For example, if your policy has a $1,000 deductible, you must pay the first $1,000 of medical expenses before the health insurance company begins to pay for any services. 

When do I have to pay for services? You, the patient, are ultimately responsible for all charges any time you receive medical care. You are expected to pay in full for your services until your deductible and any applicable co-insurance is met. 

How will I know when my deductible has been met? You can call your insurance company at any time to check on how much of your deductible has been met. Some insurance companies provide this information on-line.

How will I know how much you are going to charge me? Every time you receive medical services that we believe to be covered by insurance, you will receive notification from your insurance company with how much they paid or did not pay when they send you an Explanation of Benefits (EOB). We look at your EOB carefully to determine the amount that is to be paid by you, the patient. This is the amount that Midi Health will charge to your credit card.

What about identity theft and privacy? Under HIPAA, we are under strict rules and guidelines in terms of protecting patient privacy, and your credit card is considered protected health information. Credit card numbers are encrypted and stored securely off-site.  For your protection, no credit card numbers will be stored at our practice.  Midi Health personnel will not have access to your card; only the last 4-digits of your card number will be shown in our system.

What if I don’t have a credit card? You are welcome to leave a Health Savings Account (HSA), Flex Plan, or Debit card on file.  Midi Health requires some form of payment on file before providing services. 

When will my card be charged? We will submit your claim to your health insurance company as soon as reasonably practicable after your visit. Once your insurance company processes your claim, you and Midi Health each will receive an EOB explaining the amount for which you, the patient, is responsible.  Once Midi receives the EOB from your insurance company, Midi Health will charge your credit card for the Balance Due. We will notify you of charges greater than $25 before charging your credit card. 

How will I know that you have charged my credit card? You will receive an email receipt when your credit card is charged. 

What charges will my card be used for? Your credit card will be used only when a balance becomes due.  This may occur when Midi Health does not collect all co-pay amounts due from you at the time of service, when your insurer makes an error with regard to amounts due from you, and when additional amounts are due from you after adjudication of your claim by your insurance company.

What if my credit card is declined or expired? We will contact you to update the information in your Midi Health dashboard.  If your account becomes delinquent, you will be sent to collections. 

What if I want to change the credit card on file? You can provide a new credit card number to your Midi Health dashboard at any time. 

What if I need to dispute my bill?  Please contact Midi Health at care@joinmidi.com.  We will work with you to understand if there has been a mistake. We will refund your credit card if we determine Midi Health or your insurance company has made a billing error.
